How quote matching works
One inquiry. The information you need
to make your own decision.
Enter your ZIP and contact details. Appointment preferences are optional. Please do not send medical records or test results.
After human review, up to three relevant participating SAP providers may obtain paid access to your contact details and contact you with fees and availability, if a suitable match is available.
Ask questions, compare what is included and decide who to work with. Booking is always your decision.
Provider availability varies. A quote request does not guarantee three responses, an appointment or a return-to-duty outcome.

Understand the process
The DOT return-to-duty process involves more than an evaluation. It can include education or treatment, a follow-up evaluation and employer-directed testing.
A SAP determines the individual recommendations. No quote service can guarantee a timeline or clear you to return to work.

More about the processA Substance Abuse Professional (SAP) evaluates employees who have violated DOT drug and alcohol rules and recommends education or treatment. The SAP also assesses compliance with those recommendations.
You can request up to three quotes. The number of providers able to respond depends on your location, appointment preferences and availability. Three quotes are not guaranteed.
No. This is an independent quote-request service. SAP providers carry out their own evaluations, set their own fees and are responsible for the services they deliver.
A provider may offer a remote evaluation when it meets DOT requirements and their professional credential permits it. Confirm eligibility, location requirements and appointment format directly with the provider.
No. The SAP process, required testing and the employer’s decision are separate. A quote or SAP evaluation does not guarantee return to safety-sensitive work or employment.
